Requests for Medical Records

Many patients are unsure of what information they need to request when they require information from their medical record. Please use the below guide to help you get the information you need in the shortest time frame for the lowest cost.

Please be aware that your request may be classed as private work and could incur fee. We will advise you below if a fee is applicable.

Please also keep copies of any information we provide you as we may charge a duplicate request fee. The current duplicate request fee is £70.

PIP / Universal Credit Application

Patients applying for PIP / Universal Credit do not need to request any information from the surgery. In fact, your application form will explicitly state that you should not contact your GP for information.

If you need help remembering dates and diagnoses, you can register for online services or the NHS app and view your summary care record there.

The Department for Work and Pensions will contact the surgery independently for information from your medical record. Therefore, no further information should be required.

Taking Medication on a Plane

Most medications can be taken onto a plane with only a copy of your prescription. Therefore, most patients should not need to contact their GP.

You can view the rules about flying with medication on the government website.

Certain medications, such as insulin needles, will require a letter from the doctor for you to fly with them. Please note that letters from GP’s are classed as private work and will be chargeable based on the time it takes the doctor to complete.

If you still require a letter from the GP, please complete the medical report request form.

Proof of Diagnosis

If you require proof of your diagnosis, you can request a patient summary. This will include your current medication, known allergies, active problems, and the consultation notes from your recent appointments.

Currently patient summaries are taking 4 weeks to be ready for release.

If you would like a patient summary, please request this via the medical report request form.

Taxi Medicals

Taxi medicals are classed a private work and currently cost £200. For your taxi medical you will have an appointment with the doctor who will complete a medical evaluation and complete any forms required.

If you are having your taxi medical completed by a private healthcare provider, we can provide you a patient summary. Please allow up to 4 weeks for us to prepare you patient summary.

You can request either a taxi medical or a patient summary by completing the medical report request form.

Fitness to Work / Medical Report for Your Employer

Our doctors are not trained in occupational health. Therefore, we are unable to comment on your fitness to work. Your employer will need to contact their own occupational health provider to carry out this assessment.

If you need to provide information from your medical record for this assessment, your employer will need to request a medical report from the practice. They must have your written consent to do this and can only request specific information needed for the assessment. Your employer is not allowed to request your full medical record.

Proof of Address

The surgery does not provide proof of address letters.

We can provide a letter of registration. This will be on headed paper address to ‘Whom it may concern’ and will include your details, current address, date of registration, and a list of the appointment you have attended. The letter will be signed by an admin staff not a doctor.

Not all organisations will accept a letter of registration. Please check before requesting your letter as there is a non-refundable fee of £20 per letter per patient.

Letter of Registration

The surgery can provide a letter of registration upon request. This is classed as private work and will cost £20 per letter. Please note that each letter is for 1 patient only.

This letter will be on headed paper address to ‘Whom it may concern’ and will include your details, current address, date of registration, and a list of the appointment you have attended. The letter will be signed by an admin staff not a doctor.

Insurance Report

The surgery currently charges £104 for an insurance report. Please give consent to your insurer to request this report. This is because your insurer will be responsible for paying this if they request it. However, you will be responsible to pay the fee if you request the report.

When giving consent to your insurer you have the right to view your request before we send it. Please view your report and return it to the surgery in a timely manner as taking too long to view your report may result in your insurer cancelling your request.

Moving to a new Doctors

If you are moving to a new GP in the UK, you do not need to request your medical records as these will be automatically transferred to your new GP when you register with them.

If you are moving to a doctor outside of the UK. You can request a copy of your medical record via our website. Please view the rules regarding requesting your medical record before making your request

Medical records for court / solicitors

Medical records take about 4 weeks to be ready for release, please request them with plenty of time to spare to ensure you have all the information you need in time for court.
